In this role, you will creatively support individuals currently under the care of Blackburn, Blackpool, Chorley, Lancaster, Ormskirk, and Preston hospitals to re-join the community and achieve their goals, whilst living as independently as possible in a place they call home. The role… Our exciting, yet fast-paced Mental Health Social Care Hospital Discharge Team requires an accessible and proactive Social Worker to guide our team through challenging cases and daily tasks. This role involves working with adults with a range of mental health difficulties who require statutory interventions. You will undertake complex assessments, commission care, undertake duty tasks, and collaborate with the wider multi-disciplinary team. The post requires integrated working as part of a team, supporting Lancashire residents being discharged safely and timely from the hospital wards in Blackburn, Blackpool, Chorley, Lancaster, Ormskirk, and Preston. You will need plenty of energy and motivation, with a "get up and go" attitude to thrive in this role, along with a desire for self-development.
