Social Prescriber Link Worker, Cambridge
This role involves connecting individuals with local community resources to support their social, emotional, and practical needs. The position aims to enhance well-being through community engagement and empowering individuals to access available resources effectively.
Responsibilities include:
* Conducting assessments of clients' social, emotional, and practical needs.
* Providing personalized consultations to build trust and gather information.
* Connecting clients with local resources and activities aligned with their needs and goals.
* Maintaining relationships with community organizations to stay updated on available services.
* Developing support plans with clients and setting achievable goals.
* Offering ongoing guidance and encouragement.
* Building relationships with community groups, charities, and organizations.
* Organizing group activities, workshops, and events to promote social participation.
* Keeping records of interactions, progress, and outcomes.
* Preparing reports to demonstrate the impact of social prescribing.
Requirements:
* Relevant qualification in Social Prescribing.
* Experience in a similar role, preferably in healthcare or community settings.
* Strong communication and interpersonal skills.
* Empathy and trust-building abilities.
* Knowledge of local community resources.
* Enhanced DBS check within the last 12 months or on the update service.
