Dorset Healthcare are looking for a Community Specialist Occupational Therapist to join our supportive team based at Bridport Community Hospital, covering the Bridport and West Dorset area.
As an MDT we work with adult patients in their own homes, Community Hospitals, or Care homes to promote independence and improve quality of life. The role of Community Specialist Occupational Therapist provides specialist assessment, advice and interventions to avoid unnecessary admissions to hospital, support timely discharges and manage longer term rehabilitation for those with a long-term condition or experiencing a period of physical disability.
The role will require you to provide holistic clinical assessment and management of patients, be able to problem solve, and make decisions about their treatment. We offer opportunities for further training and development. There is also the chance to be involved in Service development projects.
This role is full time. However, part time would also be considered (16 hours Minimum) as long as the services needs are met. Current hours covered by the service are 08:00-18:00 7 days per week with weekend enhanced pay!
This post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.
Main duties of the job
The post holder would be required to provide OT based rehab for a range of physical presentations such as stroke and neurology, orthopaedics, frailty, falls, cognitive impairment and many more. Skills required to fulfil this role include knowledge and experience in moving and handling, experience of using a range of equipment, experience of carrying out cognitive assessments, skills to holistically assess and formulate a patient centred rehab program, knowledge and experience of working with the older person, particularly with frailty.
The team also provide a 7 day service to support admission avoidance for patients in their own homes. Skills required to manage this cohort of patients include the ability to recognise and manage a deteriorating patient, problem solving and crisis management of an acute situation, skills in carrying out a basic physical assessment including basic observations and recognising when it is appropriate to escalate.
The post holder will be required to work with the rest of the team to provide cover across the 7 days on a rota basis. They will also be required to carry out rehab and assessment on the rehab ward or when in community to cover duty which involves working until 18:00 to cover any urgent referrals.

Person Specification
KNOWLEDGE, SKILLS AND TRAINING
* Registered practitioner to degree/diploma level supplemented by post registration diploma level specialist training and/or short courses or demonstrable extensive experience in the relevant specialty
* Membership of the relevant Professional Body
* Learning and Assessing in Practice Qualification or equivalent practice assessors training
* Evidence of recent professional development in an up to date portfolio
* Knowledge and understanding of Trust Strategy relevant to role
JOB SPECIFIC EXPERIENCE
* Experience at Practitioner Band 5 level
* Experience of specialist working
* Recent previous experience within a comparable role
* Experience of managing change
* Demonstrable knowledge of assessment and therapeutic interventions in area of specialism
* Experience of developing specialist programmes of care for an individual or groups of patients/clients and of providing highly specialist advice

* Demonstrable skills in written and spoken English to a standard which enables the post holder to carry out the full range of duties and responsibilities of the role effectively.
