Schoen Clinic Newbridge are looking for a Social Worker who will work as a member of the multi-disciplinary team representing a care management/social work perspective to patients experiencing mental health difficulties. Their main purpose is to provide a confident experienced social work input to the team, including the development of the role and delivering expert advice and guidance on all safeguarding issues. This is a part time role working 30 hours per week. Here are some of the key things you'll be responsible for: To promote and provide expertise and guidance to the wider MDT on a range of social care interventions, which acknowledge the health and social context of the women under the care of the service. To plan and implement appropriate mental health and social care evidence-based interventions. To liaise with patients, their families, other disciplines and agencies to monitor, evaluate and modify treatment in order to measure progress and ensure effectiveness of intervention. To monitor patients on going mental state as part of the MDT. To monitor patients parenting capability in relation to their mental health and recognise any issues around safeguarding children; therefore, ensuring the wellbeing of the child always remains paramount. To make strong links with statutory, voluntary and community agencies towards greater social inclusion for patients and carers and to act as a resource for the MDT on identifying and accessing relevant agencies and unmet needs. To establish robust communication networks including verbal, electronic and written, with patients, carers, other healthcare professionals and relevant agencies. To be responsive to the needs of patients and their families and establish and maintain a therapeutic relationship with them. To promote awareness of the role of social worker within the team, negotiating priorities where appropriate. To liaise with other members of the multidisciplinary team, giving verbal and written feedback of the patient’s progress at MDT reviews and CPA meetings as well as contributing towards care plans and risk assessments. To provide specialist advice in the MDT decision making process particularly around risk assessment and safeguarding issues. Meet regularly with the Clinical Manager to discuss caseload management and identify other team business issues. To contribute towards the services clinical governance arrangements and quality agenda. To be the designated Safeguarding Lead for Schoen Clinic Newbridge. Second Safeguarding Lead is allocated within the nursing team. Work within NICE guidelines, Schoen Clinic policies and procedures, Professional Codes of Conduct and Health and Safety at Work Act. Working within a time of change, further development of Schoen Clinic’s mental health service provision. To act as an autonomous senior clinician being accountable for a defined caseload including patients with complex adult safeguarding needs. To participate in the mentorship, education and supervision of social work students on practice placement as well as other students on placement within the service. Be committed to attending clinical and business team meetings and representing the service at other meetings/ activities. To contribute to the induction and training of all staff in the hospital, including students where appropriate. Provide supervision for junior staff and peer support to other team members as appropriate. To provide clear, concise records, verbal and written reports to the clinical team, and other relevant parties, adhering to local procedures and standards as related to documentation. To undertake audits and/or research relevant to social work, disseminating findings at local level. Key Skills & Experience The successful candidate will have a d egree or equivalent in social work, c urrent social work England registration and l evel 3 safeguarding training for adults and children. Experience of multi-disciplinary working with adults with mental health conditions. Practice experience of applying assessment skills within a statutory social work setting. Knowledge of safeguarding for adults with care and support needs and children at risk. Knowledge of methods of social work intervention. Knowledge of relevant legislation, regulations guidance & policy issues. Knowledge & understanding of reporting issues to relevant external bodies. Contemporary knowledge of social work & social work applications; i.e. attachment theory, enabling change and behaviour management.
