We are seeking to recruit enthusiastic Band 5 Staff Nurses on our Elderly care assessment unit based here on ward 3 at Bradford royal infirmary within the Division of Elderly Care at Bradford NHS Trust. Our Elderly Medical Unit is based on Ward 3 and takes referrals both from AED and GP's. Here patients are admitted and assessed and usually within 24 hours are Discharged home, transferred to our downstream wards or Intermediate Care wards. We also have a Virtual ward which facilitates early discharge from hospital using the discharge to assess model of care. The post holder will be supported in their professional development and will have the opportunity to develop in leadership and management in the future. Main duties of the job The post holder will be expected to work collaboratively with AED, downstream wards, intermediate Care and the discharge team. The Elderly and Intermediate Care Service in Bradford strives to provide safe, high-quality care to the older population of the region, and is a leader in the development of safe alternatives to acute care. .We offer a consultant-led service for people over the age of 77 (or sometimes those over 65 from care facilities, depending on clinical need), who are acutely unwell. All our wards work very closely with the Intermediate Care wards, providing rehabilitation and restoring functional abilities and the Virtual ward to support older people at home.
