Job summary
We have an exciting opportunity for a Pharmacy Technician to join our Medicines Management Team based across Burnham Medical Centre and Highbridge Medical Centre. You will work alongside a team of pharmacists, pharmacy technicians, prescription clerks.
You will assist with medicines reconciliation, support drug monitoring work, and answering medication queries from patients and community pharmacies, as well as supporting the practice with all aspects of medication safety. You will liaise with GPs, practice staff, community pharmacies and secondary care colleagues, and have direct contact with patients and carers.
You will need to demonstrate the ability to use your initiative, have a positive attitude, a can-do approach towards problem solving, and be a good team player. Full training will be given.

Main duties of the job
MAIN DUTIES AND RESPONSIBILITIES
1. To carry out medicines reconciliation in accordance with practice protocols and to update patients medical records and prescriptions.
2. To carry out medication switches, dose optimisation and medicines synchronisation to ensure safe and cost-effective prescribing.
3. To initiate repeat prescribing for appropriate patients
4. To provide advice to GPs, staff, and patients in changes to prescribing in order to support the improvement of prescribing safety, quality and cost effectiveness.
5. To participate in medication audits and QOF quality improvement projects
6. To respond to medication queries from patients and staff in a professional manner referring to the appropriate GP in accordance with practice protocols
7. To support care homes in maintaining effective medicines management and reduce waste.
8. To carry out domiciliary visits where required to support patients use of medication.
9. To advise on the sourcing and safe management of medicines as appropriate
10. To action drug alerts
11. To act as a link between practice and local community and hospital pharmacy services
12. To provide information on KPIs to show benefits of the post.

JOB SUMMARY
The post-holder will support and assist activities in all of the surgeries within the South Somerset West Primary Care Network to help patients to get the best from their medicines by supporting systems to improve the continuity of care, handling of prescription queries from patients and health care professionals and supporting medication management in care homes. They will also participate in medication audits, quality improvement projects and monitoring of high-risk drugs to ensure ongoing patient safety and high quality prescribing.
This post is funded by NHS England and will include a requirement to be enrolled in, undertaking or qualified from, an approved training pathway. For example, the CPPE Primary Care Pharmacy Educational Pathway (PCPEP) or Medicines Optimisation in Care Homes (MOCH) or courses approved by the Association of Pharmacy Technicians UK (APTUK) as meeting the requirements of the UK-wide APTUK/PCPA National Competency Framework for Primary Care Pharmacy Technicians

To support the prescription clerk team, including:
25. Liaise with patients and pharmacies regarding prescription queries and requests.
26. Work with the practice medicines management teams, clinicians, and other practice staff to resolve all medicines related queries.
27. Liaise with hospital staff with reference to discharge and clinic letters.
28. Check patient records for chronic disease and drug monitoring and arrange for testing and review as appropriate.
29. Enter relevant patient information accurately onto EMIS.
30. Use the Prescription Tracker to establish the status of electronic prescriptions.
