Mewstone PCN comprises of four practices in south-west Devon working together to deliver first-class healthcare to our communities.
Main duties of the job
Clinical Pharmacy in General Practice is part of an exciting programme of transformation to develop a new model of care which addresses our ambition to deliver person-centred, co-ordinated care across our practices. Our Prescribing team aims to pro-actively help patients stay safe and well and out of hospital as well as helping to reduce the demands on general practice.
The Care Coordinator will support the role of the Prescribing Support Manager to deliver the Clinical Pharmacy in General Practice model of care, including supporting and assisting our practices.
About us
Mewstone Primary Care Network has a list size of approximately 32,000 patients. The PCN practices are Church View Surgery, Wembury Surgery, Yealm Medical Practice and Dean Cross Surgery, the practices share the same ethos of supportive learning.
You will work alongside our GPs, nurse teams, administrators and extended workforce including clinical pharmacists, pharmacy technicians, first contact physiotherapist, mental health practitioner, social link workers, health & wellbeing coaches and care co-ordinators.
All network practices are high achieving practices with a commitment to mentoring and supporting their staff.
Job responsibilities
Main Duties and Responsibilities
* Identify patients requiring medication review and arrange appropriate medication review clinics or home visits if required by the organisation/practices and liaise with carers, Community Health teams or social care staff.
* Help support the safe and effective operation of the prescribing system in the GP practice, including data reporting and project planning.
* Provide administrative support to PCN GP practices or care settings to enable them to develop, implement and monitor an agreed prescribing process plan which meets the objectives of the medicines management plan.
* Supporting GPs, care staff and primary health care teams on risk management processes relating to the prescribing and monitoring of specialist drugs e.g., controlled drugs and those subject to shared care arrangements.
* Provide administrative support to improve the disease registers within each practice to ensure all suitable patients are identified and offered suitable treatment and monitoring for their condition.
* Participate in multi-disciplinary meetings related to prescribing topics.
* Support clinical audits of prescribing in areas identified by the Prescribing Support Manager and agreed by the PCN or as directed by the PCN.
* Undertake any other duties appropriate to the grade as allocated by line management.

Person Specification
Qualifications
* Good written, oral and IT skills.
* GCSE or equivalent in Maths and English.
* Good organisational skills.
* Literate with Excel and Word.
* Computer literate with Microsoft Office packages (essential).
* Has attention to detail, able to work accurately, identifying errors quickly and easily.
* Has a planned and organised approach with an ability to prioritise their own workload to meet strict deadlines.
* An excellent understanding of data protection and confidentiality issues.
* Computer literate with an ability to use the required GP clinical systems (desirable).
Experience
* Experience of working within a healthcare setting.
* Experience of working with patients.
* Experience of working in an office-based team environment.
* Experience of working within General Practice.
* Experience working with medication prescribing processes.
