Job Title: Learning Disability Support Worker Location: Cumbria, Carlisle Salary: £11.50 per hour Job Type: [Full-time/Part-time] Overview: We are seeking a compassionate and dedicated Support Worker to provide high-quality, person-centred support to individuals. The successful candidate will work collaboratively with service users, colleagues, and external professionals to promote independence, well-being, and healthy life choices. Key Responsibilities: Service User Support: Promote the rights and responsibilities of service users. Work as an effective team member to ensure the best outcomes for individuals. Provide advice and information to service users and relevant stakeholders. Deliver tailored interventions in line with individual care plans using evidence-based models. Liaise with external professionals such as GPs and Social Services. Maintain accurate records and contribute to service monitoring requirements. Conduct risk assessments and implement effective risk management strategies. Ensure adherence to Health & Safety requirements. Assist in the facilitation of effective care pathways. Support individuals in achieving personal goals in accordance with their care plans. In residential settings, ensure service users take prescribed medication on time, providing assistance when necessary. Facilitate access to community-based services and resources to enhance service users' quality of life. Quality Assurance: Maintain high-quality standards and meet performance criteria. Participate in data collection and service improvement initiatives. Contribute to the continuous development of the service. Personal Development: Engage in regular performance management meetings to set and review objectives. Participate in relevant training and professional development opportunities. Health & Safety & Risk Management: Maintain a safe and clean working environment. Complete risk assessments where required. Take personal responsibility for health and safety by reporting concerns and ensuring necessary precautions are taken. Adhere to all health and safety policies, including serious incident and accident reporting procedures. Requirements: Strong communication and teamwork skills. Ability to follow care plans and implement effective interventions. Understanding of risk management and safeguarding procedures.
