Job Overview
We are seeking a skilled Dietician (FCP) to join our team in providing high-quality care to patients. The ideal candidate will have a strong background in Dietetics, particularly in a hospital (FCP) setting, and possess excellent patient care skills.
Key Responsibilities:
* Provide expert dietetic advice and support to patients referred through the PCN.
* Conduct thorough nutritional assessments and develop personalized nutrition plans.
* Collaborate with healthcare professionals for a multidisciplinary approach to patient care.
* Educate patients and families on nutritional principles and healthy eating habits.
* Monitor and adjust nutrition plans based on patient progress.
* Maintain accurate and detailed patient records.
* Develop and deliver health promotion and educational programs.
* Support the integration of dietetic services into the PCN.
Requirements:
* Relevant registration to practice in the UK.
* Valid undergraduate degree in Dietetics or related field.
* Minimum of 3 years of experience as a UK Dietitian, with at least 2 years of experience with PCNs.
* Excellent communication and interpersonal skills.
* Strong organizational and time-management skills.
