An exciting opportunity for appropriately qualified clinical pharmacist to work within the Primary Care setting. The successful candidate will join a group of Primary Care Network clinical pharmacists working under the supervision and mentoring of a Senior clinical Pharmacist.
The role of a Clinical Pharmacist is required to work across two GP practices in the coastal town of Redcar (Saltscar and Ravenscar). The position is an ARRS supported role for Redcar Coastal PCN.
As a Clinical Pharmacist, the post holder will work as part of a multidisciplinary team to develop and run processes for repeat prescription reauthorisation, management of medicines on transfer of care and systems for safer prescribing.
The role includes patient-facing review with the applicant undertaking clinical medication reviews with patients within the practice or domiciliary visit to proactively manage polypharmacy and produce recommendations for senior clinical pharmacist, nurses and/or GP on prescribing and monitoring.
Main duties of the job
The post holder will be required to travel independently between the 2 GP practices and attend meetings.

* Can plan, manage, monitor, advise and review general pharmaceutical care programmes for patients in core areas, including long term conditions identified by local Pharmaceutical Needs Assessment.

* Demonstrates ability to use skills in a range of routine situations requiring analysis or comparison of a range of options.
* Recognises priorities when problem-solving and identifies deviations from the normal pattern and can refer to seniors or GPs when appropriate.

Redcar Coastal PCN are able to provide the successful candidate with access to peer support and mentorship. Working across two GP Practices the post holder will establish and build effective working processes and communication pathways with all members of the primary care teams.
The successful candidate will have access to regular supervision and training provided by the PCN Senior Clinical pharmacist.
General Practice will support the candidate to foster and maintain strong links with all services across the PCN, community and hospital pharmacists, other healthcare professionals including pharmacy technicians, social prescribers and first contact physiotherapists.
Job responsibilities
Primary Responsibilities
* Manage the repeat prescribing reauthorisation process by reviewing patient requests for repeat prescriptions and structured medication reviews, reaching review dates and flagging up those needing a review.
* Undertake clinical audits of prescribing in areas directed by the GPs, feedback the results and implement changes in conjunction with the relevant practice team.
* Implement changes to medicines that result from MHRA alerts, product withdrawal and other local and national guidance.
* Auditing practices compliance against NICE technology assessment guidance.
Care Quality Commission:
Work with the general practice team to ensure the practice is compliant with CQC standards where medicines are involved.
Patient facing clinical structured medication review. Patient facing Long-term condition clinics.

Person Specification
Experience
* 2 Years post graduate experience in pharmacy.
* 2 years post graduate experience working in general practice.
Qualifications
* Completion of an undergraduate degree in Pharmacy, and registration with the General Pharmaceutical Council GPHC.
* Has an awareness of the breadth of common acute and long-term conditions that are likely to be seen in a general medical practice.
* An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing.
* The successful candidate will be required to enroll on the course within 3 months of commencing the job, to complete the CPPE Primary Care Pharmacy Education Pathway PCPEP as a requirement for this role if not already completed.
* An appreciation of the nature of GPs and general practices.
* Excellent written and verbal communication skills.

* Able to work under pressure and to meet deadlines.
* Minimum of 2 years post graduate experience in pharmacy, as demonstrated within a practice portfolio.
* May hold or be working towards a prescribing qualification.
* May hold or be working towards a postgraduate pharmacy qualification.
* A member of or working towards Faculty membership of the Royal Pharmaceutical Society.
* CPPE Primary Care Pharmacy Education Pathway PCPEP.
* In-depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.
* Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences e.g. patients.
