The role of the Team Leader is to provide high quality care and support, as defined in the care and support plan, to our clients within the clients' home, therapeutic environment or in the community.
To also manage support team to ensure day to day care and strategic outcomes are met.
48 hour shift to contain 2 x 24h shift included as a minimum. Remaining others to be allocated across the week as duties dictate:
Team Leaders are expected to:
1. Provide leadership and support for care staff
2. Provide a weekly update via email to Clinical lead & coordinator.
3. Act as a role model for good clinical and care governance practice
4. Uphold the rights, choice and dignity of Service Users.
5. Ensure Libertatem Healthcare Group policies and procedures are followed at all times.
6. Ensure Care Quality Commission standards are met for all clients.
7. Deliver a high quality care service without discrimination
8. Notify clinical lead or coordinator of any training requirements
9. Liaise with clinical lead and take lead on delivery of MDT recommendations
10. Provide leadership to the team acting as a positive role model at all times.
11. Motivate, support and mentor the staff when needed.
12. Manage rota/annual leave
13. Conduct monthly supervision, feedback and ongoing training needs
14. Hold team meetings, take minutes, and disseminate information with follow-up directed steps
15. Maintain a proactive, positive approach
16. Update care plan, risk assessments and behaviour strategy plan with incorporation of professionals' information
17. Hold monthly meetings with parents
18. Join MDTs and feedback these minutes to the team
19. Ensure medication stock is ordered
20. Conduct bi-weekly discussions with AP to check in and give her the chance to raise concerns
21. Promote a full and active life full of opportunities and choice
22. Have difficult discussions with the team
23. Lead outcome goal planning in line with activity schedule
24. Conduct monthly audits of community-based activities
25. Ensure all instructions given by therapists are embedded in AP life and updated accordingly.
26. Audit behaviour management used - restraints, de-escalation techniques etc.
27. Support independence and continued therapies
28. Promote self-care and self-management to achieve maximum independence for service users, including assistance with mobility, feeding, washing, bathing, dressing, and domestic care tasks.
29. Provide emotional support to increase Service Users' confidence and motivation.
30. Assist with the use of existing equipment to promote independence and provide access to specialist equipment as appropriate.
31. Administer medication as designated in the care and support plan in accordance with Company policies and procedures.
32. Assist Service Users with therapeutic activities as directed by the care and support plan and multidisciplinary team.
33. Deliver support sensitively to the service user's culture, disability, race, age, sexuality, marital or civil partnership status, transgender status or religion/belief.
34. Document clearly in daily records on the progress of rehabilitation programs, observing, recording, and reporting on changes in a Service User's physical and/or emotional condition.
Communication
1. Assist the healthcare coordinator and clinical lead nurse by communicating all information to ensure Service Users' needs are met.
2. Work in partnership with all members of Libertatem team and report any changes in a Service User's mental or physical health, or social conditions.
3. Ensure that concerns around safeguarding issues or equality and diversity are reported in line with Company procedures.
4. Report any serious incidents, accidents or near misses to the office team, following company procedure.
5. Report any hazards or potential hazards that impact on the safe delivery of care to the office team, in line with Company procedure.
6. Report to the office team any concerns, complaints, or suggestions for improvements raised by Service Users and/or their Carers.
7. Report to the office team promptly and appropriately to protect service users from neglect or abuse.
8. Maintain clear, concise and legible Service User records, according to Company policy and procedure.
9. Use Libertatem Healthcare's 'On Call' system for additional support if required, in accordance with Company guidance.
10. Ensure that all computer security procedures are followed.
11. Always act in a manner which respects the confidentiality of Service Users, staff and any other involved parties.
Health and Safety
Every employee has a duty under the Health and Safety at Work Act 1974, to:
1. Take reasonable care of the health and safety of themselves and all other persons who may be affected by their acts or omissions at work.
2. Co-operate with Libertatem Healthcare Group to ensure compliance with Health and Safety legislation and the Health and Safety policies and procedures by not intentionally or recklessly interfere with, or misuse, anything provided in the interests of health, safety, or welfare, in pursuance of any of the relevant statutory provision.
Data Protection and Confidentiality
The Support worker must at all times respect the confidentiality of information in line with the requirements of the Data Protection Act. This includes, if required to do so, obtain, process and/or use information in a fair and lawful way, and to disclose data or information only to authorised persons or organisations as instructed.
Training
The Team leader will complete the mandatory and client-specific training both in-house and external to achieve the specified level of competency for each client with whom they work.
Prepare for, attend and participate in regular supervision and appraisal meetings with the healthcare coordinators and clinical lead nurses.
Demonstrate a commitment to personal development.
The Team leader will be proactive in providing suggestions to clinical lead on any additional training that may benefit the team in delivering bespoke and client-centered care and support.
