Job summary The Coastal Primary Care Network is looking for an enthusiastic, self-motivated and driven Social Prescriber to join our dynamic team to work in a patient facing role. The Coastal PCN is seeking an exceptional individual to be at the forefront of this initiative. If so, we would love to hear from you Main duties of the job You will work as part of the wellbeing team to strengthen personal resilience to improve wellbeing and reduce health inequalities. You will do this by running several clinics across the PCN, offering both face to face and telephone appointments to patients of the PCN. You will promote patient choice by adopting a person-centred approach to your style of work and have an awareness of the wider determinants of health. In doing so, you will support people to have more control over their lives and help them to feel empowered to manage the complexities of their life.
