Job Overview
We are currently seeking a dedicated and skilled Band 6 Occupational Therapist for a community-based role at Leeds Community Healthcare. The successful candidate will work across various community settings, providing patient-centered care and promoting independence.
Key Responsibilities
1. Assess, plan, and deliver occupational therapy interventions for patients in the community.
2. Work with multi-disciplinary teams to support patient rehabilitation and recovery.
3. Develop and implement tailored care plans to meet individual needs.
4. Provide advice and education to patients, families, and carers.
5. Manage a caseload, ensuring timely and effective interventions.
Key Requirements
1. HCPC registration as an Occupational Therapist.
2. Proven experience in community-based occupational therapy.
3. Strong communication and interpersonal skills.
4. Ability to work independently and as part of a team.
5. Experience in managing complex caseloads is desirable.
