An exciting opportunity has arisen for a full-time Patient Pathway Facilitator within the Cancer Directorate at CUH. This is a fantastic chance to become part of a small, dynamic and friendly team dedicated to reducing patient length of stay and improving patient flow. You’ll be working closely with the multidisciplinary team (MDT) to support patients in getting home sooner, ensuring they receive the highest level of care. We are looking for an experienced nurse with a background in Oncology/Haematology, Chemotherapy, and/or Bone Marrow Transplant. If you’re passionate about improving patient pathways and enjoy working in an evolving service, this is the role for you! You’ll be working in a fast-paced and supportive environment where your clinical expertise, excellent communication skills, and patient-centred approach will make a significant impact. This is an exciting time to join the Cancer Nursing Team at CUH, as we continue to develop and expand our services in preparation for the opening of our very own Cancer Hospital (CCRH).

As a Patient Pathway Facilitator, you’ll be responsible for coordinating day-to-day discharges, supporting ward environments to ensure safe and efficient patient discharges, and working closely with junior staff to help them manage routine discharge planning and on-the-day discharges. You’ll demonstrate professional leadership, acting as a role model, and ensuring the delivery of high-quality, safe, and effective clinical care in line with the corporate nursing and midwifery agenda.
