What You Will Do:
1. Case Management: Manage a short-term caseload while effectively monitoring, prioritising, and reviewing cases.
2. Assessments: Conduct assessments under the Care Act 2014 and perform formal Mental Capacity Assessments as required.
3. Safeguarding: Respond to both statutory and non-statutory safeguarding concerns.
4. Service User Support: Work in a person-centred way to maximise individuals' strengths and assets. Provide valuable information, advice, and signposting to other services and agencies.
5. Collaboration: Offer your expertise within the Multi-Disciplinary Team (MDT) of the REaCH service and network with other professionals for effective multi-agency working.
6. Team Support: Provide guidance and shadowing opportunities for colleagues and social work apprentices. Participate in team meetings and relevant training sessions.
An in-depth understanding of adult social care laws, including the Care Act 2014 and Mental Capacity Act 2005, is crucial. You should also exhibit flexibility, creativity, and resilience when working with individuals living with risk and show competence in safeguarding practices and person-centred approaches.
Additional Requirements:
A valid driving licence and access to a car for work-related travel are essential due to the nature of the job. Social Work England (SWE) registration and experience in short-term case management within reablement or adult social care settings are also required.
