The post holder will be part of the Home Treatment Team (HTT) covering the whole of the Flintshire area, the total population being 150,000. However the HTT is also supported by 4.6 WTE community consultants who also contribute to patient management decisions of relevant patients who are already open to the CMHT. The team is well established and is multi-disciplinary. There is a team leader and a clinical lead. The post holder will work using the Care Programme Approach and New Ways of Working, providing medical leadership to the HTT for the patients referred to the team. The post holder will work accordingly to the HTT operational policy, which has been accepted by colleagues.

The maximum team caseload at any one time is 24.
The post holder will not have direct clinical responsibility for inpatients already under the care of a CMHT Responsible Clinician.
If a patient is new to service and not known to the CMHT, then the post holder will have direct clinical responsibility for the inpatient during the course of the admission. If community management is required in the future a handover meeting will take place with the CMHT at the point of discharge from hospital. The Flintshire Acute Care consultant would not have an inpatient individual case load exceeding 5 individuals at any one time.

* Collaborative management of Flintshire Service users admitted to the 36 bed inpatient unit with respective local CMHT. The unit consists of 2 wards each consisting of 18 beds. Inpatient numbers are minimal and it is extremely unlikely that the Flintshire Acute Care consultant would not have an inpatient individual case load exceeding 5 individuals at any one time.
* Attend weekly multi-disciplinary team meetings in community bases. The average length of the meeting is one hour per site and the HTT attendance and feedback is immediately prioritised.
* Management of complex cases with HTT and acute care.
* Clinical leadership within Acute Care setting.
* Offer assessment of referrals with HTT and acute care.
* Develop care plan and treatment formulation, guidance on evidence based treatment and effectiveness.
* Mental Health Act 1983 assessments when required.
* Mental Health (Wales) Measure 2010 implementation in acute care setting.
* Multi-disciplinary, multiagency and partnership working.
