Job Title: Breast Care Nurse
Overview: An exciting opportunity has arisen for the role of a Breast Care Nurse in this dynamic and challenging specialty. You will play a key role in the delivery of the Breast Care Service, supporting Breast Cancer patients throughout their cancer journey.
Responsibilities:
1. Provide a specialist and advanced nursing contribution to the clinical management of patients with breast cancer across all boundaries.
2. Develop a specialist Nurse led service for patients with a Family History of Breast Cancer and Breast Pain.
3. Supervise and lead the team with patient-centered follow-up management.
4. Form an integral part of the multidisciplinary team approach in caring for the physical, social, and psychological needs of patients diagnosed with cancer and their carers.
5. Ensure coordination and management of patients at all points of their treatment, acting as a point of contact for patients prepared for Patient Led Follow Up (PLFU).
6. Act as a resource for multi-professional teams involved in the care and management of patients with a breast cancer diagnosis.
7. Maintain and develop the Breast Care Nursing service at Walsall Healthcare Trust.
8. Attend and be a core member of the Breast MDT meetings in accordance with national guidance.
9. Manage own workload across a wide range of specialist activities.
10. Assist with the implementation of Treatment Summaries.
11. Develop and implement personal treatment plans that empower patients in their follow-up care.
12. Provide information to patients related to treatment and follow-up, signposting to appropriate healthcare providers.
13. Provide telephone advice and triage where appropriate, documenting and communicating outcomes as per service specifications.
14. Perform Holistic Needs Assessments using designated screening tools.
15. Document all interactions accurately in line with Trust policy and procedure.
16. Manage existing follow-up clinics and create new clinics within the self-supported management structure.
Qualifications:
1. Demonstrate excellent communication skills, both verbal and written.
2. Complete training requirements and competency framework to transition to a Band 7 senior clinical nurse specialist post.
