In this position, you’ll collaborate closely with a dedicated team of advisers and specialist nurses, ensuring that patients who smoke receive the highest standard of care. We are looking for someone with the passion and drive to work with a complex and often vulnerable patient group, striving to achieve the best possible outcomes. Your role will be essential in enhancing the quality of care, while also contributing to the broader mission of tackling preventable illnesses, reducing health inequalities, and easing the demand on healthcare services. You will be required to submit evidence of your qualifications for this position. If your qualifications were obtained overseas, please ensure that you acquire a Statement of Comparability from UK ENIC to facilitate a clear understanding of how your qualifications align with UK standards. Key Responsibilities: Manage a dedicated caseload, coordinating and planning care for a specific group of patients. Provide specialised advice on specific conditions and treatment pathways. Assess patients holistically, implementing and evaluating evidence-based care plans. Offer advice and support to patients, families, carers, and the wider multidisciplinary team. Deliver education, training, and expert advice to support workforce development. Promoting Health and Wellbeing: Promote the overall health and wellbeing of service users by offering advice and support for healthier lifestyles. Conduct assessments and develop personalised care plans for individuals seeking to reduce harm from tobacco. Actively support a culture shift towards a smoke-free environment. Provide advice and support to ward staff, reinforcing training on reducing harm from tobacco. Ensure robust discharge planning for patients wishing to continue treatment after discharge.
