What the job involves In this role, you will provide support and advice to care leavers, help them with financial capability, accommodation options, education, and access to healthcare. Your duties include participating in assessments and reviews of Pathway Plans, facilitating family contact, coordinating services, and monitoring progress. You will work collaboratively across various teams and agencies, attend meetings, and ensure data is accurately maintained. Continuous learning and development are key aspects of this position.
