Key Responsibilities: -Provide outstanding care and support to service users under the guidance of registered professionals. -Collaborate within a multidisciplinary team to deliver person-centred care. -Contribute to the assessment, planning, and evaluation of care plans. -Support junior team members and Trainee Assistant Practitioners. -Maintain service user confidentiality and uphold professional standards. -Promote choice, dignity, and respect in all aspects of care. Skills and Experience Required: -Level 5 Foundation Degree in Assistant Practitioner (Healthcare) -Experience working in healthcare, with a preference for those with neuro or palliative care experience. -Understanding of relevant legislation and its application in practice. -Effective communication and interpersonal skills. -Ability to work independently within the scope of practice.
