About Us The Vale Community Resource Service is an integrated health, social care and third sector team, working in collaboration with other services across Cardiff and the Vale of Glamorgan. We work with people in their own homes within the Vale locality, to maximise functional independence in activities of daily living, thus reducing the needs for admission into hospital and longer term care services. We also offer therapeutic intervention and reablement support to people following an admission into hospital, ensuring a seamless and timely transfer. We aim to provide person centred care and therapeutic intervention. We work collaboratively with the individual towards achieving goals that have been jointly identified. The team includes Occupational Therapists, Social Workers, Physiotherapists, Speech and Language Therapists, Dieticians, Nurses, Care Co-ordinating staff and Reablement Support Workers. About The Role Pay: Grade 6 (£28,334 -£31,067 per annum) plus enhancements Hours: 37 hours per week including weekend rota Main place of work: Barry Hospital. Community based working across the Vale of Glamorgan Permanent To carry out non-complex occupational therapy assessments with service users who have permanent and substantial impairments within their own home environment About You We are looking for a Social Care Officer/Occupational Therapy Assistant who is forward thinking and enthusiastic regarding innovative ways of working. You must have good communication skills and the ability to relate to and understand the needs of the person being supported. The skills to work independently, use own initiative and also as a wider multidisciplinary team is essential. You must also have a positive attitude and the motivation to enable people to remain at home and maintain function, independence and to respect their choices.
