Job Title: Qualified Social Worker - Reablement

Salary: £28.35 - £31.97 per hour (Umbrella) or £24.75 - £27.91 per hour (PAYE)
Contract: 3 Months, 22.5 hours per week including Mondays

What you will do:
1. Deliver person-centred Social Work interventions that focus on strength-based approaches and preventive strategies.
2. Work closely with service users and carers to develop and implement effective reablement plans.
3. Empower clients to identify and achieve their personal goals related to social care tasks such as washing, dressing, showering, and meal preparation.
4. Facilitate community engagement by supporting clients in accessing activities and resources to maintain their independence.
5. Collaborate with a multidisciplinary team to ensure comprehensive support and continuity of care for service users.
6. Maintain accurate records and documentation in line with industry standards and policies.
What we are looking for in a Social Worker:
You must have Social Work England registration and be eligible to work in the UK. A full UK driving licence is essential, and you must have a degree level qualification or equivalent in Social Work. #J-18808-Ljbffr
