Job Summary
Exciting Opportunity for Children & Young People Case Managers to join the ICB to develop services for Young People across Coventry & Warwickshire.
The post holder will play a key role to ensure delivery of the best possible care within available funding, representing "value for money," to support the seamless implementation of the National Framework for Children and Young People's Continuing Care (CYPCC). This role includes contribution to the development of the service, training programmes and facilitating others' learning.
Main Duties of the Job
1. Provide assessment, occasional case management and review for children with complex healthcare needs subject to Continuing Healthcare or joint funding arrangements, carrying a case load of the most complex cases.
2. Provide advice, screen referrals and co-ordinate assessments in alignment with the National Framework for Continuing Care and local policies, supporting an end-to-end Children and Young People's Continuing Care service.
3. Process and quality assure referrals/screening tools, CYPCC assessments & ongoing health reviews.
4. Work as part of the ICB Children & Young People's team to provide a case management approach by collaborating with specialists across learning disability, mental health, physical disability, and end-of-life care.
5. Facilitate the transition of young people with care packages moving from children's services to adult services.

Job Responsibilities
1. Have a working knowledge of the National Framework for Children and Young People's Continuing Care and maintain relevant and up-to-date knowledge and skills.
2. Complete assessments holistically on an individual basis within a multi-agency arena.
3. Manage Continuing Care Assessments, making individual recommendations as an outcome of assessment.
4. Undertake joint assessments with family/carers and other professionals.
5. Assess & develop support plans, implement & review individualised packages of support.
6. Secure best value for money packages while meeting clinical needs.
7. Promote physical and mental well-being and empower families in the management of their care.
8. Ensure the voice of the child and young person is heard.
9. Act as an information resource for Coventry & Warwickshire ICB.
10. Deliver and support the development of CYP Continuing Care training sessions.
11. Ensure compliance with legal requirements and statutory rules.
12. Participate in clinical audits and research-based practices.
13. Manage a caseload effectively and facilitate transitions to adult services.
14. Identify children and young people at risk and manage safeguarding issues.
15. Participate in the ICB appeals process and Continuing Care Panels.
Person Specification
Knowledge, Training, and Experience
1. Professional Registration with governing body (e.g., Registered Nurse, Social Worker, or Allied Health Professional).
2. Degree level qualification or equivalent experience.
3. Evidence of post-registration training.
4. Significant experience with children and young people with complex health needs.
5. Experience working with children & young people with Learning Disabilities, Autism & Mental Health conditions.
6. Demonstrable experience coordinating projects.
7. Knowledge of the National Framework for Children's Continuing Healthcare.
