The Primary Care Network Manager role is a fast-paced, exciting, and dynamic position existing to support the development of the PCN and lead the delivery of the Primary Care Networks' agenda. The Manager will work within the PCN Leadership team alongside the Clinical Directors and the PCN board members and will be a key link in supporting the relationships between the PCN and the member practices.
The Manager will also be required to work with other PCN Managers within the footprint of Derbyshire Integrated Care Board to support a cohesive and collaborative approach to the Primary Care agenda.
Salary: NHS Band 8a, pro rata for part-time hours.
Hours/days: 30 to 37.5 hours per week over 4 to 5 days (which must include Fridays).
Contract: Contract of Employment with Heartwood Medical Practice (Lead Practice).

Main duties of the job
The PCN Manager will be required to ensure all governance requirements are met, translate national and local policy into meaningful changes, and support practices to achieve improved outcomes for the local population while striving to reduce health inequalities.
The remit of this role is broad and will require attention to detail in the delivery of high-quality day-to-day operations at our PCN estates facility and the flexibility to support an evolving and forward-thinking Primary Care Network.

Swadlincote Primary Care Network (PCN) was formed in May 2019. The member practices are Gresleydale Healthcare Centre, Heartwood Medical Practice, Newhall Surgery, Swadlincote Surgery, and Woodville Surgery. Collectively, the surgeries serve a population of 58,000 patients across South Derbyshire.
South Derbyshire already has one of the fastest-growing populations in the region, and this is set to continue over the coming 5-10 years. Swadlincote PCN is forward-thinking and dynamic. The five member practices work collaboratively and cohesively to ensure that the PCN provides high-quality patient care within the constraints of the NHS financial budget. We undertake work at scale to provide a fair and equitable provision to our patients while ensuring resilience for the practices across the PCN footprint. We deliver clinical care from a central estates facility which currently houses our Advanced Nurse Practitioner, Paramedics, Mental Health Workers, and Social Prescribers, as well as the core PCN Administrative and Leadership Team. It is also the site from which both our Enhanced Access Service and TEAMUP service are delivered. We believe strongly in working across organisational boundaries and have good working relationships with Derbyshire Integrated Care Board, Derbyshire General Practice Provider Board, Derbyshire Healthcare NHS Foundation Trust, Derbyshire Community Healthcare Services, Derbyshire Health United, as well as social care and voluntary sector colleagues.
Job responsibilities
Job Purpose:
The post-holder will provide senior management and expertise in finance, strategic leadership, operational oversight, and workforce planning for our Primary Care Network (PCN). The role will work alongside the PCN Clinical Director and in close collaboration with other PCN Support roles to coordinate the successful achievement of all governance requirements. The PCN Manager will work with the PCN to translate national and local policy into meaningful changes, clarifying strategic objectives and priorities for the network delivery of contract requirements including PCN DES and local Enhanced Service targets. It will oversee the delivery of PCN services, aligning them with the NHS Long Term Plan objectives. Any areas of underperformance will be identified, and improvement plans implemented in collaboration with stakeholders. It will provide regular performance reports to the PCN Board and Clinical Director. The post-holder will act as a key point of contact for member practices, fostering collaboration and communication to support member practices in achieving improved outcomes for the local population with a key focus on improving inequalities in healthcare.
The post-holder will be responsible for financial oversight and will develop, manage and monitor PCN financial plans, ensuring that budgets are managed effectively and within delegated limits. It will oversee claims for funding and reimbursements, ensuring accuracy and compliance with contractual requirements. Identify opportunities for additional funding and resource allocation.
The post-holder will be required to support the Lead Practice in human resources management developing the PCN Workforce plan, supporting the recruitment, onboarding, and management of PCN staff. Working in collaboration with the Lead Practice, it will ensure that HR policies and procedures that support staff well-being, development, and retention, are developed, implemented, and followed. The post-holder will ensure that appropriate appraisals are undertaken, manage performance issues, and provide leadership to ensure a positive and productive environment.
The PCN Clinical directors have incorporated Swadlincote Primary Health Care Resources Ltd (SPHeRe) as an organisation capable of holding NHS provider contracts and directly employing staff of the PCN. At present, central PCN employed staff are under the contractual control of Heartwood Medical Practice. It is planned that in due course employment will TUPE to SPHeRe thus providing the PCN with a formal entity which all existing PCN functions and elements of this job description will fall under.
Travel between Network sites and to other venues for meetings etc., will be required from time to time.
The primary workplace will be Ragsdale House, 138 Burton Road, Woodville, DE11 7JG.
