Are you looking for a rotational post in the community in Hull and the East Riding? We are able to offer a rotational post that currently covers Hull and East Riding Community Rehabilitation and Hull and East Riding Intermediate Care. Our Community Rehabilitation team work with patients who have a variety of physical and neurological conditions, with the overall aim of them being able to maintain or improve their functional ability after illness/injury or a deterioration in their condition.
We work alongside Physiotherapists and Associate Practitioners to achieve patient goals, which can be as varied as being able to prepare a meal independently following a brain injury, to being able to get up the stairs to use the bathroom following a hip fracture.
Main duties of the job
The aim of our Intermediate Care service is to prevent hospital admissions and to facilitate discharges. We have several bedded units in the community where we work on rehab goals with patients before they go home. We also carry out rehab with people in their own homes, and we respond to crisis when people are struggling at home .
We work alongside Physios, Nurses, Pharmacists, GPs and Associate Practitioners in our MDTs. The working pattern in Intermediate Care includes working some weekends/public holidays and evenings until 8pm.
We have recently begun a cross organisational rotation with our Humber Teaching NHS Foundation Trust colleagues, therefore are very pleased to be able to support staff who are wanting to have experience in both physical and mental health. We are very happy to support and develop new staff, including those who have recently qualified and are developing their professional skills. We have a supportive Band 5 peer group who meet regularly, and an OT Professional Development group also.

Essential
* Diploma or Degree in Occupational Therapy
* Current HCPC Registration
Desirable
* Demonstrable experience as a Practice Educator of preregistration students
* Evidence of current CPD/formal qualifications at an advanced level e.g. degree or post graduate level or equivalent knowledge/experiential learning
Knowledge & Experience
Essential
* A broad understanding of relevant policy, legislation drivers and their application to clinical and service area, e.g. Mental Capacity Act, Social Inclusion
* Knowledge of Occupational Therapy models of practice, standardised assessments and their use
* Basic IT skills
* Willingness to undertake and complete preceptorship
* Appropriate knowledge/experience of working in community and adult physical health clinical practice areas
