Band 6 Clinical Nurse Specialist / AHP for Virtual Wards - North Cumbria
The Virtual Ward has been commissioned to provide a safe alternative to manage and monitor a specific group of patients with acute respiratory illness, Frailty or Heart Failure as an alternative to hospital admission and /or to facilitate earlier discharge from hospital. The patients care will be delivered in their own home by utilising cutting edge patient friendly technology to provide remote monitoring, along with additional tailored in person or telephone support.
The role of the virtual ward team is to coordinate the hospital based and community based elements of the ward by;
1. Promoting the benefits of the virtual ward to patients and HCP
2. Identifying and admitting patients to the virtual ward, ensuring seamless transfer from the hospital site to the virtual ward.
3. Assessment and remote monitoring of patients in the community
4. Delivering Specialist expertise to ensure patients feel supported on the virtual ward
5. Employing holistic assessment and strategies to manage emerging health needs.
6. Provide administrative duties to facilitate the smooth operation of the service
7. To integrate into the wider Respiratory directorate.
Main duties of the job

8. To be an integral member of the virtual ward team with partnership working across the virtual ward MDT.
9. To develop specialist knowledge, skills and experience across an array of medical conditions suitable for virtual ward management.
10. To be responsible for providing specialist advice, guidance and care to patients and their carers.
11. To hold delegated responsibility for the virtual ward patient caseload co-ordinating admissions and discharges for patients referred to the service using digital technology.
12. To assess and treat patients requiring acute level care, which may include the provision of acute therapy including oxygen, nebulizers, diuretics etc if deemed appropriate within the virtual ward setting following clear care pathways supported by a named consultant.
13. To ensure compliance with Professional body regulations and with Trust policies, procedures and protocols.
14. To maintain knowledge, skills, abilities and attitudes necessary to deliver an evidenced based service that is responsive to the changing needs of the locality population and the service.
15. To provide and participate in clinical supervision to ensure high clinical standards, and to facilitate the development of individual team members.
16. To support other members of the virtual ward as required and to deputise, where appropriate.
