Job summary An exciting opportunity has arisen at the Primary Care Network in Harrow East for an experienced paramedic to work in general practice seeing patients in surgery and carrying out home visits for housebound patients. The successful candidate will provide a paramedic service to the patients of the 3 practices within the PCN. Main duties of the job The post holders main responsibility will be to work within the clinical team to provide high quality patient care. The Community paramedic will provide clinical assessment and management for patients of all ages with undifferentiated and undiagnosed problems seeking a primary health care service, including minor illness face-to-face consultations, home visits, care home visits and telephone triage and long term condition management The community paramedic will have ideally have experience of treating common minor illnesses in primary care The community paramedic will carry out home visits in the locality for patients who are frail or housebound and be able to diagnose and manage patients in the community or identify those requiring other community services/hopsital admission The community paramedic will have responsibility for certain care homes for which they will be primarily responsible for clinical care, including weekly care home rounds, including working with the care home lead GP, clinical team including practice pharmacists, community health and social care team members and care home staff.
