Job OverviewINTERACT (Interact Responsive Anticipatory Care Taskforce) provides both proactive and reactive care and support for adults (18+) whose health and well-being are adversely affected by their housing situation or home environment and are experiencing one or more of the following; frailty, multi-morbidity or complex needs. You will play an integral role by supporting individuals and their families in accessing support services within their local community.Objectives for this role:· Provide detailed information, advice and guidance using motivational interview techniques, that will be delivered in person, by telephone and email.· Work closely with clients to develop a Personalised Care and Support Plan (PCSP) understanding their needs, the desired outcome and the necessary interventions to provide support.· Signpost and make appropriate referrals to voluntary sector services, community initiatives, and relevant public sector services to achieve agreed actions and outcomes.· Offer short-term support to clients, fostering independence, confidence, and skill development· Be part of a multidisciplinary team focused on supporting individuals whose housing situation impacts their health and well-being positively, providing practical and holistic assistance.What we’re looking for:· Clear communicator and ability to engage with individuals from diverse backgrounds.· Ability to work collaboratively in a multidisciplinary team environment.· Willingness to adapt to changing organisational needs and priorities.· Knowledge of voluntary and community services in Norfolk and how to support people to access them· Driver’s Licence and Car is essential.A full training program will be given.
