Job summary Working Full time : 37.5 hours per week We have an exciting opportunity to join our diabetes team at the Countess of Chester to work in our inpatient diabetes service along side an experienced band 7 nurse, This post is to support patients on our ward areas with Diabetes related issues, education for patients, there relatives and our ward teams. The role will cover all aspects of inpatient diabetes care. The post holder will act as a Key Worker and a specialist resource of current clinical information addressing the specific needs of individual patients through direct/indirect interventions.The post holder will possess a critical understanding of detailed theoretical and practical knowledge in their specialist area of practice. They demonstrate initiative and are creative in finding solutions to problems and have some responsibility for team performance and service development. They consistently undertake self-development and liaise across all care settings, referring patients to other relevant Health Care Professionals, when appropriate. A key objective of the role will be to work as part of and contribute to the development of the multidisciplinary team. The post holder would support the development of nurse led clinics and existing clinics and also expand and support the CNS team Main duties of the job Plan/organize own workload accordingly. Attend wards/departments to assist and work closely with the MDT to carry out consultations and treatments. Promote and develop the specialist advisory and supportive role of a Clinical Nurse Specialist (CNS) in Diabetes, demonstrating specialist clinical skills and knowledge base supported by post registration education and qualification. Encompassing nursing care needs, including where necessary palliative and end of life care. To undertake extended roles as the service and individual needs dictate with full support and training from senior staff.
