We are seeking an Experienced Adult Social Worker to join our dedicated team committed to enhancing the lives of adults in need of social support services. Responsibilities
Conduct thorough assessments of clients' needs and situations.
Develop and implement individualized care plans in collaboration with clients and their families.
Provide counseling and support to clients facing various challenges such as mental health issues, addiction, and social isolation.
Advocate for clients within various community resources and social services systems.
Monitor clients' progress and adjust care plans as needed to ensure optimal support and outcomes.
Work closely with interdisciplinary teams to coordinate services and resources for clients.
Maintain accurate and timely documentation of case notes, assessments, and service plans.
Facilitate support groups and workshops to empower clients and promote social engagement.
Requirements
Master's degree in Social Work (MSW) from an accredited institution.
Licensure as a Clinical Social Worker (LCSW) or equivalent certification.
Minimum of 3 years of experience working as a social worker in an adult or community setting.
Strong understanding of mental health, substance abuse, and social service systems.
Excellent interpersonal and communication skills to build rapport with clients and stakeholders.
Ability to work independently and manage a varied caseload effectively.
Proficient in case management software and electronic documentation processes.
Hours Per Week: 37.00
Start Time: 09:00
End Time: 17:30
Pay Per Hour: £35.00
Location: Liverpool, Merseyside
