We are currently looking to hire a Housing Coordinator - Extra Care to join Torus! The Housing Coordinator will play a vital role
in assisting and empowering residents to maintain independent living.
This is a 12-Month Fixed Term post based at The Maples, Liverpool, one of our purpose-built extra care schemes commissioned by the
local authority to deliver 24/7 care and support to people aged 55+.
Responsibilities:
* Conduct property viewings, support tenancy management, and assist with settling in.
* Develop personal plans, make referrals, and collaborate with agencies on changing resident needs.
* Ensure smooth tenant sign-ups, arrange adaptations, and assist with assistive technology.
* Monitor health, wellbeing, and personal plans, involving families in care planning.
* Promote skill development, community engagement, and independence.
* Work collaboratively on data, audits, and performance, with flexibility across schemes.
Skills & Experience:
* Experience with older/vulnerable individuals in housing or social care.
* Skilled in assessments, support plans, caseload management, and multi-agency work.
* Knowledge of crisis management, risk minimisation, and resilient in difficult situations.
* Knowledge of Safeguarding.
Interview Process:
Candidates will undergo in-person interviews, consisting of competency-based questions.
