Key Responsibilities: In this role, you will work in front door admission avoidance, medicine, and rapid response areas, supporting discharge planning and pathway decision making. You will conduct thorough equipment assessments to ensure patients have the necessary tools for their recovery. Additionally, you will develop and implement tailored treatment plans, maintain accurate patient records, and collaborate with a multidisciplinary team to deliver superior patient care. Qualifications and Experience: • HCPC registration. • Bachelor’s degree in Physiotherapy or a related field. • Extensive experience in front door admission avoidance, medicine, and rapid response areas. • Exceptional analytical and problem-solving abilities. • Unwavering attention to detail and accuracy. • Proven ability to work both independently and collaboratively within a team.
