Key Responsibilities:
1. Conduct assessments to determine individual needs and develop personalised care plans.
2. Provide support, advice, and interventions to individuals, carers, and families to promote independence and well-being.
3. Work within the Care Act 2014, Mental Capacity Act, and safeguarding procedures to ensure legal compliance and best practice.
4. Collaborate with health professionals, housing services, voluntary organisations, and other key stakeholders to provide holistic support.
5. Monitor and review care plans to ensure they remain effective and responsive to changing needs.
6. Manage a caseload, prioritising tasks effectively to meet deadlines and service objectives.
7. Advocate on behalf of service users, ensuring their voices are heard in decision-making processes.
8. Keep accurate and up-to-date case records and reports in line with professional and legal requirements.
9. Participate in team meetings, supervision, and continuous professional development activities.
Essential Requirements:
1. Degree or equivalent qualification in Social Work.
2. Social Work England registration (or eligibility to register).
3. Experience in adult social care, mental health, or disability services.
4. Strong knowledge of relevant legislation, including the Care Act 2014 and safeguarding policies.
5. Excellent communication, assessment, and problem-solving skills.
6. Ability to work effectively both independently and as part of a multidisciplinary team.
7. A commitment to anti-discriminatory practice and promoting equality and diversity.
8. Proficiency in using case management systems and IT software.
9. Full UK driving licence and access to a vehicle (if required for the role).
