This position has become available due to our Lead Practice Nurse sadly announcing her retirement. We are looking for an experienced and enthusiastic lead nurse to support and manage a dedicated nursing team of Nurses and HCAs across both our surgeries. The lead nurse will ensure high quality patient care is provided and drive clinical excellence. You will develop staff potential and motivate your team through mentoring, appraisals and learning events. You will contribute to quality improvement initiatives. Main duties of the job
The post holder is responsible for ensuring the delivery of safe and effective Nursing Care. As the overseeing manager for the Practice Nursing Team the role will provide clinical direction, leadership, mentoring and will aid these professionals in developing their careers and knowledge. The role will include oversight of clinical policies and procedures, infection control and day to day delivery of clinical care. This is a brilliant opportunity for either a Nurse Lead looking to develop and grow or for an experienced Practice Nurse looking to take the next step in their career within an ambitious and innovative practice.
About us
The Practice operates across two sites and has a growing patient list of over 15,000 with 4 Partners and 3 salaried GPs. The GPs are also supported by 1 ANP, 1 Practice Nurse, 1 Physician Associate, 1 Paramedic, 2 HCAs, 2 Clinical Pharmacists, a Mental Health Nurse and a First Contact Physio.
The Practice achieved a Good rating in all 5 service questions in our most recent CQC inspection (September 2017). We are a training practice and usually host at least two trainees. We offer a number of enhanced services. Both surgeries benefit from ample free parking which is available to all staff and patients.
The Practice is an active member of Gravesend Central PCN and works closely with member practices and our local GP Federation. Pelham Medical Practice is working towards relocating to a new purpose built surgery in Gravesend. The relocation we hope will happen in the next 2 to 3 years and will allow us to look to service over 20,000 patients in 22 clinical rooms with modern meeting and training facilities that will allow us as a practice to further develop and grow both our clinical and non-clinical teams.

You will be required to:
1. Support the development, mentoring and professional development of others to maximise staff potential in the team.
2. Identify the potential for service developments, risk and deficits and inform practice manager making recommendations based on specialist knowledge.
3. Undertake one-to-ones, appraisals, regular Practice Nurse and HCA meetings, clinical education, and other practice-wide learning events and meetings.
4. Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF.
5. As the Clinical lead, have overall line management responsibility for the Practice Nursing Team.
6. Play a key role in our clinical governance and overall practice management team.
7. Act as a clinical leader in the delivery of practice nursing services to patients, ensuring that the needs of the patient are the priority.
8. Attend regular practice meetings.
9. Lead the nursing team in the planning and implementation of local guidelines, protocols, and standards, as well as local projects or initiatives.
10. Ensure health and safety and infection control are high priorities, supported by other members of the team.
11. Work with our Practice Management team to ensure we monitor practice performance of QOF and LCS regularly to achieve high standards of patient care.
12. Take responsibility for own development, learning, and performance, including organising clinical supervision where appropriate and acting as a positive role model.

16. Administer medication for patients in accordance with PGDs and own scope of practice.
17. Provide guidance to patients in the use of prescribed and over-the-counter medications regarding side effects and dosages.
18. Oversee the provision of wound care services to patients.
19. Co-ordinate the provision of travel medicine services.
20. Request pathology services as necessary.
21. Process pathology results as required.
22. Maintain accurate clinical records in conjunction with existing legislation.
23. Ensure read codes are used effectively by all members of the nursing team.
24. Chaperone patients where necessary.
25. Prioritise health issues and intervene appropriately.
26. Support the team in dealing with clinical emergencies.
27. Implement vaccination programmes for adults and children and ensure our care homes and house bound patients are vaccinated in a safe and timely manner.
28. Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.).
29. Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual).
30. Support the clinical team with all safeguarding matters in accordance with local and national policies.
31. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately.
32. Deliver opportunistic health promotion where appropriate.
33. Be responsible for ensuring maximum optimisation of use of appointments across the nursing team.
34. Develop and review audit protocols and processes for the practice, effectively utilising the audit cycle and oversee those within the team.
