The post holder will work as an autonomous practitioner following professional codes of conduct and ethics carrying a varied caseload which can include patients with complex needs with additional supervision. Clinical activities can take place in a wide range of settings including hospital, clinic and community settings, and patients' own homes.
Main duties of the job
JOB SUMMARY/JOB PURPOSE:
The post holder will contribute to service developments within their department which can include attending meetings and participation in audit, service evaluation and researching evidence based practice. Work in partnership with patients and other health professionals as appropriate to achieve their optimum health outcome. The post holder will be required to work closely with all service areas as appropriate to ensure the delivery of a fully integrated service.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
