JOB TITLE: Social Worker – School Program (LMSW or LCSW)
REPORTS TO: Director of School Program
HOURS/STATUS: Full Time /Exempt (10 month and 6-week ESY)
SALARY RANGE: $28.84 - $33.84
JOB SUMMARY
Under the direction and supervision of the Director of School Program, the Social Worker will provide individual and group counseling services in a variety of settings as outlined on the students Individualized Education Plans.
ESSENTIAL DUTIES AND RESPONSIBILITIES
* Functions as a member of clinic and educational interdisciplinary teams in order that services may be coordinated and delivered in an efficient and effective manner.
* Facilitates social emotional learning groups within classroom setting.
* Serves as an advocate, as needed for students/individuals.
* Initiates and facilitates referrals to community agencies.
* Completes mandated documentation pertaining to services provided.
* Obtains social history for new entrants to Holy Childhood programs.
* Updates social history at regular intervals.
* Actively participates in the planning and implementation of an ongoing program for parents of students who are transitioning to adult life.
* Assists families with legal guardianship forms and information regarding it.
* Assists with the development of a library of resources and information on services, programs, agencies, etc. for families and students/individuals.
* Represents the Agency at outside meetings, workshops, and seminars.
* Attends Agency in-service programs, staff meetings, and workshops as directed.
* Assists in the development and implementation of behavior management plan.
* Maintains accurate daily notes and completes in a timely manner
* Works with educational team to update PLP’s, IEP Goals, and transition planning.
* Provides crisis intervention and debriefing when needed.
* Sits on assigned agency committees.
* Serve as Agency Investigator and investigates incidents as requested.
* Demonstrates sensitivity to and respect for the diverse cultural backgrounds and practices of students, staff, and families of Holy Childhood.
* Provide efficient system for addressing the needs of students and implementing individualized interventions, including intensive interventions as needed. Able to utilize positive school-wide, classroom, and small-group behavioral supports.
* Other Duties as assigned.
QUALIFICATIONS
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Candidates for this position will have: the ability to demonstrate and provide examples of continuous personal development and improvement; an individual focused attitude; Communication skills (verbal and written); Problem solving skills, strong organizational skills and the ability to effectively manage multiple tasks/projects.
EDUCATION and/or EXPERIENCE
LMSW required. 2-5 years of experience since graduation; previous experience working with individuals with developmentally disabilities and exemplary communication, organizational and facilitation skills strongly preferred. LCSW preferred
