Job summary The post holder will work as a Pulmonary Rehabilitation Practitioner (Physiotherapist, Exercise Physiologist, OT, Nurse) assisting in the assessment, planning, delivery and evaluation of evidence-based pulmonary rehabilitation for patients with chronic lung disease within Liverpool Heart and Chest Hospital and in a number of community settings within Liverpool, and within patients own homes. This is a fixed term post for 18 months due to additional investment and funding. Main duties of the job To undertake respiratory assessment/treatment delivery as a senior member of the Breathe Team delivering Pulmonary Rehabilitation within the hospital, community venues, and patients own homes. To support the team leader and deputise in their absence as required. To deliver specialist clinical care in a safe and effective way, using the physical skills and techniques gained through knowledge and experience. To ensure appropriate pathways of communication and care via liaison and referral to other agencies as required. To undertake all aspects of clinical duties as an autonomous practitioner, including professional and legal accountability and managing clinical risk for all aspects of own work. Aspects of work will include delivery of treatment as an individual practitioner or as part of a multi-disciplinary team. To undertake assessment of patients with diverse or complex physical and/or psychological, conditions in order to deliver appropriate rehabilitation. To supervise students and junior staff in their treatments and to ensure on going education in all aspects of treatment and care. To ensure that treatments are evidence based and in line with current practice. To use your own car, have business insurance and must drive to community venues and patient homes etc. Optional participation in the on-call and weekend rota within the hospital. This is not a mandatory requirement for the post.
