We have an exciting opportunity for a Social Worker (must have completed ASYE and registered with Social Work England) who is highly motivated to work within the Medway Shared Lives service. The service is registered with the Care Quality Commission and currently rated as Good.
You will be involved in the recruitment of new carers, complete their assessment, training and approval. Once approved you will provide the ongoing support and monitoring needed to maintain the standards required and achieve the positive outcomes needed.
The service provides short- and long-term placements within family homes for people who have care and support needs, taking a highly individual approach to care provision. This role is strongly relationship-based and you will work closely with the Shared Lives carers to support them to provide enabling and person-centred approaches to the support they provide to people using our service.
Your practice will have a strong emphasis on standards of care and working to provide the best possible outcomes for people. We promote individual aspirations, independence skill development and wellbeing to enable people to live the lives they choose.
You will work closely with other members of the team and colleagues in ASC locality teams.
There is an expectation that the post holder will be office-based for a minimum of 2 days each week.
