Our Community Mental Health Service at Market Drayton Cottage Hospital is looking to recruit a Community Psychiatric Nurse.
You will be responsible for working in an identified adult community mental health pathway, with the flexibility of working across pathways, depending on skill and service need. You will be responsible for care coordination of an identified caseload.
In this position, you will undertake clinical assessments in accordance with the clinical pathway to establish the person's needs, goals, risks, and strengths to generate a formulation-based care plan agreed with all involved. You will deliver specialist interventions in accordance with an agreed care plan to an identified group of people experiencing mental health-related difficulties.
You will also be expected to provide consultation, support, and clinical supervision to other health professionals working within the Community Mental Health Pathway Service and other areas of the Trust as identified.
You will be working in an identified adult community mental health team, with the flexibility of working across pathways, depending on skill and service need. You will deliver a high-quality clinical service to patients and their families while managing a complex caseload and being responsible for clinical and caseload supervision of other team members where identified.
Main duties of the job
1. Take responsibility for managing a complex caseload, which includes specialist assessment (within service specification), care planning in conjunction with service users and carers, and evaluation of individual treatment plans (Care Programme Approach and Single Assessment Process).
2. Participate in setting quality standards, including auditing, monitoring, and reviewing practice in line with current clinical guidance and policy.
3. Administer medication, complying with relevant Trust procedures and NMC Guidance.
4. Assess patients' individual care needs, develop, implement, and evaluate programmes of care.
5. Deliver a range of interventions in accordance with the agreed plan of care and Pathway guidance, including pharmacological and psychologically informed interventions.
6. Complete and contribute to robust risk assessments and risk management plans. Deliver and promote positive risk-taking, where appropriate, to improve quality of life and maximize occupational performance/independence.

Person Specification
ESSENTIAL CRITERIA
* A working understanding and application of delivering interventions for mental health.
* Qualified in a therapeutic mode of intervention.
* Experience of delivering Supervision or Qualified as a Placement Assessor.
ESSENTIAL CRITERIA
* Qualified CPN or Social Worker.
* Registered professional with Professional Body.
* Minimum of 3 years experience post-qualified.
* Experience of working within a Community Mental Health setting.
* Experience of working within a Care/Treatment service with either the NHS or Local Authority.
