Job Summary
At Falcon-Care, we are thrilled to offer an incredible opportunity for an experienced Complex Care Nurse to become part of our team. This role is perfect for a registered nurse who is caring, compassionate, and motivated to provide exceptional care within the community. As this position involves supporting individuals in their own homes, regular travel will be a key aspect of the role.
You’ll be joining Falcon-Care during an exciting period of growth, as we expand our services to better meet the needs of our community and help people receive the highest standard of care in the comfort of their homes.
This position requires a strong understanding of anatomy and a commitment to promoting health and wellbeing among patients. The ideal candidate will possess excellent communication skills and the ability to work collaboratively with other healthcare professionals.
Key Responsibilities
1. Clinical Care Delivery
o Assess, plan, implement, and evaluate care for patients with complex and multiple health needs.
o Deliver specialist nursing interventions, including wound care, medication administration (e.g., intravenous therapy), and palliative/end-of-life care.
o Perform comprehensive health assessments, including physical, psychological, and social factors.
o Monitor patients' progress and adapt care plans in response to changing conditions.
2. Multidisciplinary Collaboration
o Liaise and coordinate with other healthcare professionals, including GPs, therapists, social workers, and hospital teams, to ensure integrated care.
o Act as a key worker for patients with long-term or complex conditions, facilitating regular team meetings to review care plans.
3. Patient Advocacy and Education
o Support patients and families in understanding diagnoses, treatments, and care options.
o Promote self-care and independence through education and health promotion.
o Advocate for patients' preferences and rights within the healthcare system.
4. Leadership and Supervision
o Provide guidance and mentorship to managers and staff.
o Contribute to the development of care protocols and best practices within the team.
o Participate in training sessions to enhance team skills in managing complex cases.
5. Quality Assurance and Compliance
o Ensure adherence to relevant clinical guidelines, policies, and legal requirements.
o Maintain accurate and up-to-date patient records.
o Contribute to audits and quality improvement initiatives to enhance service delivery.

Qualifications and Experience
* Essential:
* Registered Nurse (Adult, Learning Disability, or Mental Health) with valid NMC registration.
* Significant post-registration experience in community or complex care.
* Experience in managing patients with long-term conditions, palliative care, or advanced wound care.
* Desirable:
* Postgraduate qualification in a relevant specialty (e.g., Community Nursing, Palliative Care).
* Experience in leadership or mentorship roles.
* Training in advanced clinical skills, such as venepuncture, catheterization, or PEG feeding.
Additional Information
* A valid UK driver’s license and access to a vehicle may be required.
* Flexibility to work evenings, weekends, and on-call shifts, as necessary.

This role is integral to supporting patients with complex needs in the community, ensuring they receive high-quality, compassionate care while promoting independence and quality of life.

Pay: £34,000.00-£38,000.00 per year
