An opportunity has arisen for a Clinical Pharmacist to join South Uttlesford PCN on a fixed one year contract, to provide high quality patient focussed pharmacy service to the patients of South Uttlesford.
Main Duties
1. Patient facing long-term condition clinics to review the ongoing need for each medication
2. Patient facing Clinical Medication Reviews
3. Patient facing Care Home medication reviews
4. Patient facing and telephone medicines support
5. Unplanned hospital admissions – Review of medications and reconciling of medications
6. Repeat prescribing
7. Identification of high risk patients
About the Candidate
As a minimum, you will hold a Degree in Pharmacy (Masters or equivalent), be registered with the General Pharmaceutical Council and a member of the Royal Pharmaceutical Society. Ideally, you will be a non-medical independent prescriber.
You will be required to lead by example, setting and maintaining high standards of professionalism within a team environment. You should be IT literate and confident in using practice systems.
