Job responsibilities
Social prescribing empowers people to take control of their health and wellbeing through referral to non-medical social prescribing link workers who give time to focus on what matters to me. This role specifically works with young people, taking a holistic approach connecting them to community groups and statutory services that provide emotional and practical support.
You will work with other agencies and partners to support care experienced young people aged sixteen to twenty-five, and those at risk of being not in employment, education, and training, to improve their wellbeing, help them to live independently, and improve their health outcomes by connecting them into a broad range of services and interventions. Reasons for referral may include, but are not limited to, mental health difficulties, financial support, isolation, family issues, friendship issues, support with navigating the healthcare system, anxiety, and low mood. The post holder should have experience working within or alongside the care system, or with young people who are care experienced. A good understanding of mental health challenges and trauma-informed care is essential. The work will be led by the wants and needs of the young people, with some flexible working hours to fit their availability.
The Young Peoples Social Prescribing Link Worker will collaborate closely with partners across Hull, including the Leaving Care Team, Connexions, Hull City Council, Public Health, and others.
Key Areas of Responsibility
* Act as the main point of contact for young people accessing the service.
* Accept referrals and assess how far a patient's health and wellbeing needs can be met by community services.
* Manage and prioritize your caseload based on patient needs.
* Create personalized care and support plans addressing health and wellbeing needs.
* Develop trusting relationships by giving time and focus on what matters to the young people.
* Evaluate progress along the patient's journey to ensure needs are met.
* Refer patients back to referees with signposting suggestions where needed.
* Liaise with statutory and voluntary sector agencies, including education, mental health, primary and secondary care, social care, and youth offending teams, to promote timely support access.
* Work systemically, involving parents, carers, educators, and health professionals when appropriate.
* Build strong relationships with internal and external services, promoting the social prescribing model.
* Collaborate with VCSE organizations and community groups for referrals.
* Ensure data is captured accurately in clinical systems using SNOMED codes.
* Discuss concerns with relevant GPs to ensure safeguarding procedures are followed.
* Attend multidisciplinary team meetings as required.
* Ensure young people are informed about key processes and their preferences for information sharing.
* Participate in monthly supervision, mandatory training, and ongoing professional development.
* Perform other duties as directed by the line manager.
