Working with Continuing Healthcare, contracting, commissioning and clinical colleagues, the post holder will support independent providers to improve the delivery of quality care. Key responsibilities include monitoring data and supporting homes to improve services and report performance within the ICB. You will need to be experienced in working with older people and/or community settings and educated to degree level or have similar experience.
