Job Title

Responsibilities
* Plan and implement personalized care to provide residents with security, dignity, and independence.
* Build strong relationships with residents and their families, ensuring tailored care plans are met.
* Lead a team of Care Assistants, maintaining high standards of care and influencing the team's approach.
* Assist Registered Nurses and Care Managers in providing physical and mental stimulation through social and recreational activities, as well as administering medication.
* Keep accurate records of daily tasks and foster collaboration within the team.
Requirements
* Hold a Level 2 Diploma S/NVQ in Adult Care or equivalent.
* Have experience in person-centered care planning and knowledge of caring for individuals with dementia.
* Demonstrate exceptional communication skills, empathy, and a natural ability to work effectively with people.
* Be able to manage time efficiently, prioritize tasks, and maintain accurate records.
