Description The Service & Team: The Personalisation Support Team is part of the Disabled Children & Therapy Service (DC&TS) a specialist, multi-disciplinary service which supports disabled children, young people and their families whose main need for services arises from their disability, where this has a substantial or critical impact on the quality of the child or young person’s life and / or the lives of their families. The service intervenes where children’s needs cannot be fully met by universal services alone; it includes a statutory social care service that undertakes assessments that provides services at the acute and complex level of need. The Role: We are seeking a permanent Personalisation Support Advisor to join our busy, friendly team. We provide a wide range of support to eligible children, young people and their families who may be considering or have chosen to arrange their own support funded from a direct payment, to meet their assessed social care needs. The successful candidate will provide information, advice and support on the set up and ongoing management of a direct payment, this includes providing information to families about becoming an employer. The role involves visiting young people and parents or carers in their own homes, covering areas in the east and mid of the county. There is also a high level of contact both by telephone and email. The role involves regular reviews, making sure direct payment funds are being used appropriately to meet the identified care needs. You will work closely with a range of professionals, such as social workers, family workers and finance colleagues. The role is based at Chy Trevail in Bodmin, but as in line with the Council’s hybrid working, there will also be working from home and from other locations. This is a fabulous opportunity and a very rewarding role. We see first-hand how direct payment support can make a positive difference to the lives of children, young people and their families. This is a public/customer-facing role, where the statutory English language requirement for public sector workers applies. This position will be subject to an enhanced criminal record disclosure check. Working Pattern: 37 hours. We are open to discussion about flexible working. What you’ll need to succeed: Experience of working with disabled children, young people and their families is preferred. You will be reliable, organised, and have a flexible ‘can do’ attitude, be self-motivated and have a strong focus on delivering the best service to our families. You will have excellent communication skills, be numerate and accurate and have the ability to manage your own time effectively. The successful candidate will work on their own and as part of our busy team and have a professional approach applying discretion and diplomacy. Knowledge of direct payments is desirable but this is not essential as full training will be provided. You must have access to a vehicle to be able to travel across the county.
