We are looking for a fixed Term Shared Lives Coordinator within our Shared Lives Stoke Service.
The Shared Lives Stoke Service matches people who need care and support with an approved carer who shares their family and community life and gives care and support to the person with care needs.
Responsibilities:
1. Recruit, assess, support, and deliver training of new carers, providing the specialist knowledge they need to offer the right support to those placed in their family.
2. Match people into shared lives placements, monitoring and supporting these placements.
3. Develop and support effective promotional and recruitment campaigns.
4. Contribute towards general service development, shaping our services for the future.
5. Participate in the ‘Out of Hours’ support system for carers and respond to crises and emergencies.
6. Support and supervise carers, ensuring they are engaged, enabled, empowered, valued, and motivated to provide the best possible care to the people they support.
7. Organise and attend support groups for approved carers as required.
