The Advanced Nurse Practitioner will be part of the Stokewood Surgery team and will support in the delivery of nursing services, delivering care within their scope of practice to the entitled patient population.
The post holder will deliver, and lead on the delivery of, high quality safe and contemporary care, ensuring consistently high stands of practice and clinical excellence, including promoting population health, improving personal outcomes, contributing towards service evaluation, white leading and developing research and quality improvement initiatives.
Main duties of the job
Thesuccessful candidate is an experienced, enthusiastic and highly motivated ANP.Who can demonstrate autonomous and critical thinking and is a caring,compassionate and committed experienced nurse who, acting within theirprofessional boundaries, will provide care for the presenting patient frominitial history taking, clinical assessment, diagnosis, treatment andevaluation of care.
Theywill demonstrate safe, competent clinical decision-making and expert care,including assessment and diagnostic skills, for patients within the generalpractice. They will communicate and work collaboratively with the generalpractice team to meet the needs of patients, supporting the delivery of policyand procedures, and providing nurse leadership as required.

Winchester Rural South Primary Care Network (PCN) comprises four GP practices: Stokewood, Twyford, Wickham, and Bishops Waltham surgeries. Together, they provide personalised, high-quality healthcare services to the rural communities in North and Mid Hamphire.
Stokewood Surgery, part of this PCN, is located in Fair Oak, Eastleigh. It offers general practice services and contributes to the PCN's collective efforts to improve healthcare access and delivery in the area
Job responsibilities
* Assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team.
* Manage undifferentiated undiagnosed condition and identify red flags and underlying serious pathology and take appropriate action.
* Use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice.
* Actively take a personalised care approach and population centred care approach to enable shared decision making with the presenting person.
* Complete the relevant training in order to provide multi-professional clinical practice and CPD supervision to other roles within primary care, for example first contact practitioners and the personalised care roles.
* Duties may vary depending on the practice need, but will algin in accordance to the Primary Care Network Directed Enhanced Service contract.
Person Specification
Experience
* Management of patients with complex needs.
* Clinical examination skills.
* Understanding of the implications of national priorities for the team and/or service.
* Excellent communication skills at all levels in varying communication formats.
* Experience in working with clinical systems such as EMIS
Qualifications
* Independent Non-Medical Prescriber Qualification V300.
* Advanced Nurse Practitioner MSc or completed the Centres ePortfolio supported Route.
