To work as part of an integrated health and social care team providing a crisis intervention and early supported discharge service for patients across the City of Southampton registered with a Southampton CCG GP practice.
Main duties of the job
1. To have day-to-day responsibility for a designated caseload of clients requiring nursing, rehabilitation, care management intervention and to organise and prioritise the required programmes of treatment.
2. Work within the community and in UHS ED as a lone practitioner managing situations of risk with telephone support.
3. Seek support and advice with cases from more experienced colleagues when faced with matters outside sphere of responsibility, knowledge, or skills.
4. Obtain informed consent from clients to treatment, and work within agreed guidelines for clients unable to give informed consent.
5. Obtain consent to the sharing of information and document all consent issues in accordance with relevant Trust and Local Authority procedures.
6. Undertake generic health and social care assessments using the Single Assessment Process, and undertake specific comprehensive nursing assessments for clients by using investigative and analytical skills for those with acute, complex, chronic or palliative care needs.

Qualifications
* Level 1 Registered Nurse
* Degree or willingness to undertake
* Teaching and Assessing in Practice/Mentorship
* Post Registration Qualification e.g. rehabilitation, chronic disease management, palliative care, history taking, Community Nurse Specialist
* Nurse Prescriber
Experience
* Experience of working with vulnerable adults with complex presentation
* Significant range of relevant healthcare experience in either hospital or community setting including working autonomously
* Supervision and training of others e.g. staff, students
* Working within a multi-disciplinary team

Salary: £37,338 to £44,962 a year based on full-time hours.
