Job summary
We are currently recruiting for a full time (12 months fixed-term, Maternity Cover) CAMHS Transitions Worker to work in our Richmond Tier 3 Child and Adolescent Mental Health Service (CAMHS) in the London Borough of Richmond.
The post holder will support families, work alongside teams and liaise with multi-agencies, ensuring wider system collaboration to wrap around support for our young people.
These innovative roles have been developed to ensure our young people moving from CAMHS services into Adult services are sufficiently prepared and supported to navigate adult provision and networks, whilst maintaining their progress and promoting independence.
The aim of the CAMHS Transition post is to support the successful transition of young people aged 17 into appropriate mental health services and to enhance the social and mental health outcomes of young people experiencing difficulties as they grow into adulthood.
Main duties of the job
It is acknowledged that the transition from child to adult services creates risks for young people who may fall through the systems due to not having the right support. This can result in poor mental health and social outcomes, leading to loss of human potential and increased burden on health and social care services over a lifespan.
This post will directly inform and update Transition Protocol between CAMHS and AMHS based upon the latest evidence of best practice. The post holder will also be responsible for supporting teams and provide training, where necessary, to educate and skill up professionals and stakeholders in the needs of those young people moving from CAMHS into adult services. Core to the role with be ensuring excellent collaboration between the young person and families to support the young person's journey across services.

About the location:
Livingston House, Teddington
Our community hub is near shops, restaurants and stations in Teddington with trains to Richmond, Kingston, Clapham Junction, or Waterloo. This service will permanently re-locate to a new bespoke CAMHS building at Richmond Royal Hospital Spring 2025. Richmond Royal Hospital is located in Richmond Town Centre. It is a 5-minute walk from Overground and South West Train routes.

Clinical
To provide leadership to safeguarding issues to CAMHS teams for cases in process of transitions
To contribute to specialist assessments of young children and their parents/career referred to the services based upon the appropriate use, interpretation, an integration of complex data from a variety of sources including psychological and where appropriate, self-report measures, rating scales, direct and indirect structured interviews in line with CYP IAPT methodology.
Support evaluation and decision-making process for transition and treatment interventions considering both theoretical and therapeutic models and highly complex factors concerning historical and developmental processes that have shaped the individual and the family
Exercise autonomous professional responsibility contributing towards assessments, discharge and transition of young people to adult mental health services whose problems are managed by psychologically based standard care plans.
To contribute to and/or use evidence-based, integrated working tools and processes, to ensure young children and their parents/carer receive holistic and coordinated services in response to their mental health needs.
To contribute to risk assessment(s) and risk management(s) for young children and parents/carer.
Communicate in a skilled and sensitive manner, information concerning the assessment, formulation and treatment plans of young children and their parents/carer.
To maintain accurate and accessible health care records according to Trust policy an Information Governance Department.
To assist CAMHS teams in the coordination of CPA meetings and to participate in patients CPAs and other agency reviews
To take a lead role in child protection, consulting with and advising colleagues in accordance with policy and legislative frameworks
To lead on MCA 2005 MHA 1983, Care Act 2014 and safeguarding issues in relation to transition
To frequently manage complex and emotionally charged meetings with children and families where clients may be hostile, antagonistic or have high expressed emotion.
To be responsible for managing and prioritizing own workload of children and adolescents involving self-harm
To be responsible for implementing a range of group work interventions focusing on life skills training, motivational and creative activities, systemic, psychological and psychotherapeutic interventions for individual children, carers, families and groups.
To make appropriate formulations, that may need to consider risk such as sexual abuse, drawing upon a wide experience and knowledge base, maintaining a number of provisional hypotheses and adjusting and changing treatment options.
To be responsible for recording clinical work and communicating, both orally and in writing, in a skilled and constructive manner highly complex or distressing information, for example regarding the experience and effects of anorexia and bulimia on a young persons physical and developmental status.
To share assessment and treatment plans, using sensitive and age-appropriate language, being mindful that the information may be unwelcome.
To be mindful of the needs of children, young people and families from a wide range of racial, cultural, sexual religious and social backgrounds, and to develop with colleagues innovative ways of meeting these needs.
Service Development:
To be responsible for identifying desirable service developments in accordance with needs of Young People and their carers transitioning to adult services.
To contribute to the development, evaluation and monitoring of transition services operational policies and services, as directed, through the deployment of professional skills in research, service evaluation and audit.
To contribute to a culture that is therapeutic, productive, accessible, inclusive and flexible to meet the needs of the local population.
Teaching, Training and Supervision:
To receive relevant clinical and managerial supervision from relevant senior nurse and service manager, as relevant.
To gain additional experiences and skills relevant to the profession and/or the service (as agreed with the service manager) in accordance with the CAMHS directorates expectations.
To develop skills in the area of professional post-graduate teaching, training, and supervision and to provide supervision to other MDT staffs psychological work as appropriate.
To offer training relevant to the above to service users carer and colleagues.
To provide advice, consultation and training to staff working with the client group across a range of agencies and settings.
Research and Service Evaluation:
To utilise theory, evidence-based literature, and research to support evidence-based practice in individual work and work with other team members.
To undertake appropriate research and provide advice to other staff undertaking research.

Qualifications
Essential
1. Educated to degree level in a health or social care discipline
2. Registered with an appropriate professional body (, First degree in MH nursing and/or other relevant health profession
3. Professional registration with NMC or HCPC or Social Work England SWE
4. Evidence of relevant professional development

5. Further post registration training in child and adolescent mental health
6. Supervision training
7. An appropriate professional qualification to supervise students in training.
8. A postgraduate qualification in a therapeutic modality relevant to child mental health.
