Licensure:
Current Licensure as a Social Worker in the State of Maine
Knowledge, Skills & Abilities:
1. Ability to provide individualized, patient-centered care planning
2. Ability to identify social determinants of health (SDOH) impacting the health and care planning for each patient
3. Knowledgeable of the types of care that can be safely provided in a community setting and providing this information to patients and families
4. Understanding of healthcare settings and resources in the community, such as outpatient clinics, skilled nursing, or acute rehabilitation hospitals, home care
5. Understanding and educating patients and their families of their right to choose their post-acute services and documentation of providing choice
6. Monitoring and updating of the patient’s condition for necessary changes in the post-acute transition plan
7. Ability to use electronic communication software(s)
8. Leadership and negotiation skills are essential in coordinating care within the hospital and the community
Duties and Responsibilities:
1. Ability to conduct an assessment of patient needs, identify resources required to transition patients to the next setting, and secure the necessary resources
2. Collaborate, educate, and lead an interdisciplinary team of professionals in daily care rounds/care conferences/care planning.
3. Excellent communication skills in written, verbal, and electronic systems such as email or with software applications currently used with the hospital
4. Ability to work with community organizations to secure durable medical equipment, health insurance authorizations, arrange transportation
5. Participate in the length of stay and inpatient hospital population management.
6. Knowledge of hospital reimbursement methods and quality measures applicable to the care management department.
